Area Agencies on Aging
Area Agencies on Aging are local chapters of a federally-funded program (USAging) that provide a wide range of services for older adults. These are public or private non-profit agencies, designated by the each state to address the needs and concerns of all older persons at the regional and local levels. “Area Agency on Aging” is a generic term—specific names of local AAAs may vary. AAAs are primarily responsible for a geographic area that is either a city, a single county, or a multi-county district.
Services typically include low and no-cost caregiver support, case management, food and nutrition, transportation, elder abuse resources, legal assistance, etc.

Senior Centers
Senior Centers are community facilities designed to provide programs and services specifically for older adults. They often offer social activities, exercise classes, health screenings, educational programs, and other resources tailored to the needs and interests of senior citizens. Many include activities and services for persons living with dementia and their care partners.

Day Respite Programs
Choice in Aging
A nonprofit organization serving Contra Costa, Napa, Sacramento, and Solano counties. Their programs include Alzheimer's Day Care Resource Centers and Caregiver Support groups (in person) in Pleasant Hill and Antioch.
Licensed and supervised social adult day program serving people with memory loss due to mild to moderate Alzheimer’s disease or related dementias. Also provides caregiver support.

A local (Pleasanton) agency that offers individualized and customized placement services & resources. This includes Assisted Living, skilled and rehabilitative services, in-home care, respite care, and hospice care. Their services are designed to meet unique needs and preferences.

Transportation
County Connection - Central Contra Costa County
County Connection provides fixed-route and paratransit bus service.
Dial-A-RIde Paratransit Tri Valley
Provides accessible door-to-door transportation to eligible people with disabilities in Livermore, Pleasanton, Dublin, and the surrounding unincorporated areas of Alameda County. This service is available whenever fixed-route service is operating.
Dial-A-Ride is public shared ride transportation. Drivers can provide assistance to the vehicle from the front door of the origin and to the front door of the destination but do not enter private businesses or residences.
East Bay Paratransit is a public transit service for people who are unable to use regular buses or trains, like those operated by AC Transit and BART, because of a disability or a health-related condition. East Bay Paratransit transports riders from their origin to their destination in vans equipped with a wheelchair lift.

Medi-Cal
Some transportation services may be covered for those with Medi-Cal as their healthcare provider. Medi-Cal members with full scope eligibility may be eligible for free transportation to and from medical appointments, which includes: Medical appointments (including family planning), mental health, and substance use disorder services; dental appointments; picking up prescriptions; and picking up medical supplies and equipment. Most rides must be scheduled one week in advance. This service is NOT for hospital emergency room visits.

Some Senior Centers provide low and no-cost transportation services. Check with your local senior center to see if this service is available in your area.
Provides transportation, same-day prescription delivery, and home delivery for groceries and over-the-counter items.
Wheels Bus Service - Pleasanton, Dublin, Livermore
Medical Transport (private pay)
Individuals pay out-of-pocket for non-emergency transportation (medical taxis, wheelchair vans, or ambulances) for patients who require assistance getting to and from medical appointments, treatments, or other healthcare-related destinations. In most cases, this type of transportation is not covered by Medicare or other insurances. If someone is being discharged to a skilled nursing facility or acute rehab facility after an in-patient hospital stay, always check with the discharge planner to see if transportation services will be covered by Medicare.

Medicare has a data base of companies that are Medicare-approved providers of medical equipment and supplies.
If a person is being discharged home after an inpatient hospital stay, durable medical equipment (DME) that is necessary for recovery may be covered by Medicare as part of the Home Health benefit. Check with the discharge planner to see if medical equipment will be covered by your insurance after a hospital stay.
